What is hypertension?

Hypertension or high blood pressure, sometimes called arterial hypertension, is a chronic medical condition in which the blood pressure in the arteries is elevated. This requires the heart to work harder than normal to circulate blood through the blood vessels. Blood pressure is summarised by two measurements, systolic and diastolic, which depend on whether the heart muscle is contracting (systole) or relaxed between beats (diastole) and equate to a maximum and minimum pressure, respectively. Normal blood pressure at rest is within the range of 100-140mmHg systolic (top reading) and 60-90mmHg diastolic (bottom reading). High blood pressure is said to be present if it is persistently at or above 140/90 mmHg.
Hypertension or high blood pressure is sometimes called "the silent killer" because HBP has no symptoms, so you may not be aware that it's damaging your arteries, heart and other organs. Uncontrolled high blood pressure can injure or kill you. When blood pressure readings rise to 180 or above for the systolic - top - number OR 110 or above for the diastolic - bottom - number, inform your doctor or call for emergency medical treatment immediately. The possible health consequences that can happen over time when high blood pressure is left untreated include:

  • Damage to the heart and coronary arteries, including heart attack, heart disease, congestive heart failure, aortic dissection and atherosclerosis (fatty buildups in the arteries that cause them to harden)
  • Stroke
  • Kidney damage
  • Vision loss
  • Erectile dysfunction

Generic Bystolic (Nebistar Tablets) – Product Information

Generic Bystolic (Nebistar Tablets) is a beta-adrenergic blocking agent used for the treatment of hypertension, to lower blood pressure. Lowering blood pressure reduces the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions.
The active ingredient present in Generic Bystolic is Nebivolol Hydrochloride. Nebivolol HCl lowers blood pressure (BP) by reducing peripheral vascular resistance, and significantly increases stroke volume with preservation of cardiac output.
Nebivolol Hydrochloride is available as Bystolic, Nebistar, nebilong,  Hypoloc, Lobivon, Nebilet, Nebilox, Nobiten, and Temerit.

Generic Bystolic (Nebistar Tablets) - Contraindications

Generic Bystolic (Nebivolol Hydrochloride Tablets) is contraindicated in patients with

  • Hypersensitive to Nebivolol Hydrochloride or any component of the product
  • Severe bradycardia
  • Heart block greater than first degree
  • Cardiogenic shock
  • Decompensated cardiac failure
  • Sick sinus syndrome (unless a permanent pacemaker is in place)
  • Severe hepatic impairment (Child-Pugh > B)

Generic Bystolic (Nebistar Tablets) - Dosage

The adult dosage for Generic Bystolic (Nebistar Tablets) is given below:
Hypertension
The dose of Generic Bystolic has to be individualized as per the needs of the patient. For most patients, the recommended starting dose is 5 mg once daily, with or without food, as monotherapy or in combination with other medicines. For patients requiring further reduction in blood pressure, the dose can be increased at 2-week intervals up to 40 mg.
Renal Impairment
In patients with severe renal impairment (ClCr less than 30 mL/min) the recommended initial dose is 2.5 mg once daily; titrate up slowly if needed.
Hepatic Impairment
In patients with moderate hepatic impairment, the recommended initial dose is 2.5 mg once daily; titrate up slowly if needed. Generic Bystolic has not been studied in patients with severe hepatic impairment and therefore it is not recommended in that population.

Side Effects of Generic Bystolic (Nebistar Tablets)

The most common side effects reported in patients treated with Generic Bystolic (Nebistar Tablets) Bradycardia, Diarrhea, Nausea,Fatigue,Chest pain,Peripheral edema,Headache, Dizziness, Insomnia, Dyspnea, and Rash
The uncommon side effects include asthenia, abdominal pain, hypercholesterolemia and paraesthesia.

Treatment of Schizophrenia & Bipolar I Disorder with Generic Zyprexa

Generic Zyprexa Tablets (Oleanz Tablets) – Product Information

Generic Zyprexa (Oleanz Tablets, manufactured by SUN Pharma) is used for the treatment of schizophrenia (a mental disease that causes disturbed or unusual thinking, loss of interest in life, and strong or inappropriate emotions) as well as acute treatment of manic or mixed episodes associated with bipolar I disorder and maintenance treatment of bipolar I disorder in adults and adolescents between the ages of 13 to 17 years.
It is also used as an adjunct to valproate or lithium in the treatment of manic or mixed episodes associated with bipolar I disorder in adults. Generic Zyprexa (Oleanz Tablets) is used in Combination with Fluoxetine for the treatment of depressive episodes associated with bipolar I disorder.
The active ingredient present in Generic Zyprexa is Olanzapine. Olanzapine belongs to a class of medications called atypical antipsychotics. It acts by altering the activity of certain natural substances present in the brain.

Generic Zyprexa Tablets (Oleanz Tablets) - Contraindications

Generic Zyprexa is contraindicated in patients with a hypersensitivity to Olanzapine or any other ingredient present in the product.

Generic Zyprexa Tablets (Oleanz Tablets) - Dosage

Oleanz comes as an oral tablet in of 2.5 mg, 5 mg, 7.5 mg, 10 mg and 20 mg dosage forms. Generic Zyprexa (Oleanz Tablets) should be administered on a once-a-day schedule without regard to meals. The recommended adult Oral dosage of Generic Zyprexa for adults is given below:

Dosage for Treatment of Schizophrenia in adults:
Start at 5-10 mg once daily;
Target: 10 mg/day within several days
Dosage for Treatment of Schizophrenia in adolescents:
Oral: Start at 2.5-5 mg once daily;
Target: 10 mg/day

Dosage for Bipolar I Disorder (manic or mixed episodes) in adults:
Start at 10 or 15 mg once daily

Treatment of Asthma and COPD Using Generic Advair Inhaler

Generic Advair Inhaler (Seroflo Inhaler) – Product Information

Generic Advair Inhaler (Seroflo Inhaler, manufactured by Cipla Ltd.) is a combination products which contains a corticosteroid and a Long-acting beta 2-adrenergic agonists (LABA) indicated for treatment of asthma in patients aged 12 years and older. It is also used in the treatment of Chronic obstructive Pulmonary Disease (COPD).
The active ingredients present in Generic Advair Inhaler are Fluticasone Propionate and Salmeterol Xinafoate.
Fluticasone Propionate and Salmeterol Xinafoate inhaler is marketed under trade names such as Advair, Seretide, Seroflo, Viani, Adoair, Quikhale SF and Foxair.

Dosage And Administration

Generic Advair Inhaler (Seroflo Inhaler) comes as an inhaler in 50, 125 and 250 mcg dosage forms.
Seroflo inhalers contain Salmeterol Xinafoate 25 mcg (fixed) and the quantity of Fluticasone Propionate is 50 mcg, 125 mcg or 250 mcg. Seroflo inhaler contains 120 metered doses. Seroflo multihalers contain Salmeterol Xinafoate 50 mcg (fixed) and the quantity of Fluticasone Propionate is 250 mcg or 500 mcg. Seroflo multihaler contains 30 metered doses. The recommended dosage of Generic Advair Inhaler for adults is given below:
Dosage for Treatment of Asthma: Adults and Adolescents (12 years and older):
Seroflo 50 Inhaler: Two inhalations twice daily
Seroflo 125 Inhaler: Two inhalations twice daily
Dosage for Treatment of Asthma: Children (4 years and older)
Seroflo 50 Inhaler: Two inhalations twice daily
Not recommended for children below 4 years of age.
Dosage for Treatment of COPD: Adults:
Seroflo 125 inhaler: Two inhalations twice daily
Seroflo 250 Inhaler: Two inhalations twice daily

Generic Advair Inhaler (Seroflo Inhaler) - Contraindications

Generic Advair Inhaler (Seroflo Inhaler) is contraindicated in patients with a hypersensitivity to Salmeterol Xinafoate or Fluticasone Propionate or any other ingredient present in Seroflo Inhaler. It is also contraindicated in persons with a severe hypersensitivity to milk proteins. Primary treatment of status asthmaticus or acute episodes of asthma or COPD requiring intensive measures using Seroflo Inhaler is also contraindicated.

Generic Advair Inhaler - Warnings and Precautions

Before using Generic Advair Inhaler (Seroflo Inhaler) please inform your doctor of your medical history specially with regard to the following:
current/past infections (such as tuberculosis, herpes), high blood pressure, bone loss (osteoporosis), heart disease (such as chest pain, heart attack, irregular heartbeat), overactive thyroid (hyperthyroidism), diabetes, eye problems (such as cataracts, glaucoma), seizures, liver disease.
The other warnings and precautions are:

  • Salmeterol may cause a condition that affects the heart rhythm (QT prolongation). QT prolongation can infrequently result in serious (rarely fatal) fast/irregular heartbeat and other symptoms (such as severe dizziness, fainting) that need medical attention right away.
  • LABA increase the risk of asthma-related death and asthma-related hospitalizations. Seroflo should not be used in combination with an additional medicine containing LABA because of risk of overdose.
  • Seroflo Inhaler should not be used to initiate treatment in acutely deteriorating asthma or COPD or to treat acute symptoms.
  • Patients should be monitored periodically as Candida albicans infection of the mouth and pharynx may occur. Patient have to be advised to rinse his/her mouth with water without swallowing after inhalation to help reduce the risk.
  • Patients should be monitored for signs and symptoms of pneumonia as there is increased risk of pneumonia in patients with COPD.
  • Potential worsening of infections (e.g., existing tuberculosis; fungal, bacterial, viral, or parasitic infection; ocular herpes simplex). Use with caution in patients with these infections. More serious or even fatal course of chickenpox or measles can occur in susceptible patients.
  • If paradoxical bronchospasm occurs, discontinue Seroflo Inhaler (Generic Advair Inhaler ) and institute alternative therapy.
  • Risk of impaired adrenal function when transferring from systemic corticosteroids.
  • Hypercorticism and adrenal suppression may occur with very high dosages or at the regular dosage in susceptible individuals. If such changes occur, discontinue Seroflo slowly.
  • Seroflo is to be used with caution in patients with cardiovascular or central nervous system disorders because of beta-adrenergic stimulation.

Side Effects of Generic Advair Inhaler (Seroflo Inhaler)

Generic Advair Inhaler (Fluticasone Propionate and Salmeterol Xinafoate) may cause side effects. The common posssible side effects reported in patients treated with Generic Advair Inhaler (Seroflo Inhaler) include runny nose, sneezing, sore throat, throat irritation, sinus pain, headache, nausea, vomiting, diarrhea, stomach pain, muscle and bone pain, dizziness, weakness, tiredness, sweating, tooth pain, red or dry eyes, shaking of a part of your body that you cannot control and sleep problems.
Some side effects can be serious. If you experience any of the following side effects, call your doctor immediately: coughing, wheezing, or chest tightness that begins soon after you inhale fluticasone and salmeterol, hives, rash, swelling of the face, throat, tongue, lips, hands, feet, ankles, or lower legs, choking or difficulty swallowing, hoarseness, noisy, high-pitched breathing pounding fast, or irregular heartbeat, fainting, chest pain, cough, burning or tingling in the hands or feet, blurred vision, white patches in the mouth, fever, chills, and other signs of infection.

Treatment and Prevention of Breast Cancer using Nolvadex Tablets

What is Breast Cancer?

Breast Cancer is a type of cancer originating from breast tissue, most commonly from the inner lining of milk ducts or the lobules that supply the ducts with milk. Cancers originating from ducts are known as ductal carcinomas, while those originating from lobules are known as lobular carcinomas.
Ductal carcinoma in situ (DCIS), or intraductal carcinoma, is breast cancer in the lining of the milk ducts that has not yet spread to nearby tissues. It may progress to invasive cancer if untreated. Lobular carcinoma in situ (LCIS) is a marker for an increased risk of invasive cancer in the same or both breasts.
The size, stage, rate of growth, and other characteristics of a breast cancer determine the type of treatment. Treatment may include surgery, drugs (hormonal therapy and chemotherapy), radiation and/or immunotherapy.
Some breast cancers are sensitive to hormones such as estrogen and/or progesterone, which makes it possible to treat them by blocking the effects of these hormones.

Nolvadex Tablets (Tamoxifen Citrate) – Product Information

Nolvadex Tablets contains the active ingredient Tamoxifen Citrate which works by blocking the actions of oestrogen, a female sex hormone. Nolvadex Tablets are effective in the treatment of metastatic breast cancer in women and men.
Adjuvant Treatment of Breast Cancer. Nolvadex is indicated for the treatment of node-positive breast cancer in postmenopausal women following total mastectomy or segmental mastectomy, axillary dissection, and breast irradiation.
In women with Ductal Carcinoma in Situ (DCIS), following breast surgery and radiation, Nolvadex is indicated to reduce the risk of invasive breast cancer. Nolvadex is indicated to reduce the incidence of breast cancer in women at high risk for breast cancer.
Tamoxifen is used to treat infertility in women with anovulatory disorders. Tamoxifen is used to prevent estrogen related gynecomastia, resulting from elevated estrogenic levels. It is taken as a preventative measure in small doses, or used at the onset of any symptoms e.g. nipple soreness/sensitivity.

Nolvadex Tablets (Tamoxifen Citrate) - Contraindications

Nolvadex Tablets is contraindicated in patients that have hypersensitivity to Tamoxifen Citrate or any inactive component of this medicine. It is also contraindicated in women who are pregnant or intend to become pregnant within two months after treatment. Women who are breastfeeding or intend to breastfeed, cannot be administered Tamoxifen Citrate as it is unknown if tamoxifen passes into breast milk.

Nolvadex Tablets (Tamoxifen Citrate) - Dosage

Nolvadex Tablets can be taken with or without food at approximately the same time each day. Swallow the tablets with a glass of water. The dosage varies as per the indication being treated. The recommended dosage of Nolvadex tablets for various indications is given below:
Breast Cancer:
For patients with breast cancer, the recommended daily dose of Nolvadex is 20 mg to 40 mg. Dosages greater than 20 mg per day should be given in divided doses (morning and evening).
Ductal Carcinoma in Situ (DCIS):
The recommended dose is 20 mg of Nolvadex daily for 5 years.
Reduction in Breast Cancer Incidence in High Risk Women:
The recommended dose is 20 mg of Nolvadex daily for 5 years.
Infertility (Anovulatory Disorders):
The recommended dosage of Nolvadex Tablets (Tamoxifen Citrate) is 10 mg to 40 mg per day which is is administered in days 3 to 7 of a woman's cycle.

Side Effects of Nolvadex Tablets (Tamoxifen Citrate)

Inform your doctor if you notice any of the following and they worry you: hot flushes, nausea, vomiting, any changes in your period, unusual/changes in vaginal discharge, itching around the genital area, headache, dizziness or light-headedness, diarrhoea or constipation, skin rash, fluid retention, leg cramps and unusual hair loss or thinning. These are the milder side effects of Nolvadex Tablets.
Inform your doctor immediately in case you notice any of the following:

  • Abnormal vaginal bleeding
  • Unusual pain or pressure around your pelvis, in your bones or anywhere in your body
  • Excessive thirst
  • Swelling of the hands, ankles and feet
  • Lower stomach pain or discomfort.
  • Pain and reddening around the tumour
  • Unusual tiredness, shortness of breath and dizziness when exercising, and looking pale
  • Problems with vision
  • Unexplained bruising
  • Frequent infection, fever, severe chills, sore throat or mouth ulcers.
  • Yellowing of the skin and eyes, and dark coloured urine.
In case of unusual vaginal bleeding or other gynaecological symptoms such as pelvic pain or pressure when you are taking Nolvadex or anytime afterwards, inform your doctor. This is necessary as changes to the lining of the womb may occur, some of which may be serious. Most of the above side effects are rare however you may need urgent medical attention.
Stop taking Nolvadex Tablets and inform your doctor immediately or go to your nearest accident and emergency if you experience any of the following symptoms:
  • An unusual cough
  • Severe pain in the chest which may reach the left arm
  • Breathlessness
  • Any unusual, severe or prolonged headache or migraine attack
  • Partial or complete loss of vision, or double vision
  • Slurring or speech disability
  • Sudden changes to your hearing, sense of smell, or taste
  • Dizziness or fainting
  • Weakness or numbness in any part of your body
  • Severe pain in your stomach
  • Severe pain or swelling in either of your legs
Increase risk of blood clots and ovarian cysts (in pre-menopausal women) have been seen with Nolvadex Tablets. A blood clot is a very serious side effect. You may need urgent medical attention or hospitalisation. Do not be alarmed by this list of possible side effects. You may not experience any of them.
